Chief commissaires: please use the Excel invoice. It does the arithmetic for you and avoids errors.

- USA Cycling First Report of Occurrence (PDF) — if a rider is hurt at the event. Send it to USA Cycling with the rider’s signed waiver within 24 hours. If this link doesn’t work, the current form is on USA Cycling’s event organizer resources page.
